Assisting You With Registering A Death

The loss of a loved one is a shock under any circumstances, and the amount of administration involved in registering a death may seem like a daunting task.

You will not be alone, we will be by your side navigating you through the whole process and offering you advice and support every step of the way. 

When A Death Occurs

In The Hospital

If your loved one passes away while in hospital, a member of the hospital staff will reach out to the next of kin. Your loved one will be cared for in the hospital’s mortuary until arrangements are made to bring them into our care.

The hospital team will gently guide you through the next steps, including how to obtain the Medical Certificate of Cause of Death. Once we are notified, we’ll begin making the necessary preparations, supporting you with compassion and care throughout the process.

At Home

When a loved one passes away at home, we understand how emotional and overwhelming those first moments can be. When you’re ready, please contact us directly. We will arrange for your loved one to be brought into our care with dignity and compassion.

Once they are with us, we will notify the appropriate medical centre of their passing and coordinate the necessary steps to ensure the Medical Certificate of Cause of Death is completed and forwarded to the medical examiner.

Throughout this process, we are here to support you every step of the way with sensitivity, guidance, and care.

In A Nursing Home Or Hospice

If your loved one passes away in a nursing home or hospice, the staff will guide you through the initial steps and advise you to contact us so we can bring your loved one into our care with dignity and compassion.

In a hospice setting, the team will typically handle the Medical Certificate of Cause of Death and forward it to the medical examiner. They will also provide you with a bereavement booklet containing helpful information and support.

In a nursing home, the staff will inform us directly when your loved one is ready to be brought into our care. They will also let us know which medical centre to contact so that the Medical Certificate of Cause of Death can be arranged.

Planning All Aspects Of A Funeral

In certain circumstances a death may be referred to the coroner. This means you will not be issued with the Medical Certificate of Cause of Death immediately and will not be able to register the death until the coroner gives their approval.

Registration

When someone sadly passes away, a Medical Certificate of Cause of Death must first be completed. This is issued by a doctor and sent directly to the medical examiner for review.

Once the certificate has been approved by the medical examiner, it is then forwarded to the appropriate local registrar. At this stage, you can make an appointment to officially register the death. This can usually be done online or by telephone.

There are several registration offices across Lancashire, including Fleetwood, Lancaster, Preston, and St Annes. If your loved one passed away in Blackpool, the registration will need to take place at Blackpool Town Hall.

Registrar Procedure

The registrar will ask several questions in order to record the death effectively. This will include:

  1. The full name and address of the deceased

  2. The place and date of birth

  3. The date and place of death

  4. The deceased’s last known occupation

  5. Whether the deceased was receiving any state benefits or pension

  6. In the case of the deceased being a married female, her maiden name

  7. The full name, address and occupation of the person registering the death

Official Documents and Administration

You will receive a number of official documents and paperwork which may seem confusing at first. However, Moons Funeral Service are at hand to guide you through the entire process. We can help you with filling in any forms, or simply be there to provide emotional support.

These documents may include:

1. A Green Certificate, which is needed for a burial or cremation to take place

2. Certified copies of the entry of death

3. BD8 Form, supplied by the Department of Work and Pensions and concerning the state pension of the deceased

4. Bereavement Registration Form, which will help to remove the deceased’s name from mailing lists and protect against identify fraud

5. Information booklet “What to do after a death” issued by Job Centre Plus, which will become an invaluable point of reference through the coming weeks
